[Vois] Integration Environment milestone

OAS Group and Oracle Group have completed their work for us and installed our application.  Joe in particular has been hard at work correcting problems and shepherding our change requests through the deployment system.

We have completed the steps for initial system configuration<https://fogbugz.mc.vanderbilt.edu/default.asp?W102> to allow the development team to log in and check the system.  You can access the system here<https://jam04lt.mc.vanderbilt.edu:9015/PatientPlanManagement/> or here<https://jam06lt.mc.vanderbilt.edu:9015/PatientPlanManagement/> (these are temporary, please do not bookmark them); a problem with the F5 prevents the main address from working (see below).

The operation of the application in INT is completely consistent with operation in DEV, although we have not quite achieved the ability to edit patient plans (see below).

The open issues are these:

 *   The internal patient list in INT is empty, so no new plans can be created because there are no patients for whom to create them
    *   The solution is to implement the features discussed on Tuesday to populate the internal patient list as new patients are requested (e.g. via StarPanel).  Joe and Eric are making progress on this today.  This will be available when we release the next version to INT.
 *   The OAS Group may disable our application at an indeterminate time in the future, if we exceed our logging quotas which is likely to happen.
    *   THe solution is to remove all unnecessary logging and set appropriate logging levels throughout the application.  This will be available when we release the next version to INT.

 *   The F5 load balancer is not working as expected.  The application cannot yet be accessed via https://voisint.mc.vanderbilt.edu.
    *   We will ask the Operations team to investigate this.  The temporary workaround is to access the application either here<https://jam04lt.mc.vanderbilt.edu:9015/PatientPlanManagement/> or here<https://jam06lt.mc.vanderbilt.edu:9015/PatientPlanManagement/>.
